Astellas has established Quick Start for AAP applicants who have an immediate need for Prograf. Quick Start offers several benefits:

• Prescreening for patient eligibility
• Quick delivery time so patient can start therapy as soon as possible
• Shipment of Prograf within one business day after receipt of the completed Quick Start Form
• Direct-to-patient shipping
• Allows the patient and provider more time in completing the AAP application

To begin the prescreening process, call 1-800-477-6472 and provide the following information:

Facility name Physician’s name, DEA, and state license number Office contact’s name, phone, and fax number Name of the requested product Confirmation that Prograf is prescribed for an appropriate FDA approved indication OR one that is indicated in authorized medical compendia	Name, date of birth, and social security number Home address and phone number Confirmation of the patient’s insurance status Household size and income Verifiable shipping address in the US Transplant history Annual income

If the patient qualifies for Quick Start based on the prescreening process, Astellas will fax a Quick Start prescription form and a partially completed AAP application to your facility. Complete the Quick Start prescription form immediately and fax it back to 1-866-317-6235. An initial 30-day supply of Prograf will be delivered directly to the patient within 2 business days.

Please note: Quick Start approval does not mean that the patient has been enrolled in the Astellas Access Program^SM. Unless the patient qualifies and is enrolled in the Astellas Access Program^SM, no further shipments past the initial 30-day supply will be sent. The patient and provider must complete and submit the full formal AAP application in order to be enrolled.
