The Astellas Access ProgramSM (AAP) provides Amevive at no cost to qualified patients who are uninsured or underinsured. This program is managed by Astellas Reimbursement ServicesSM (ARS).
Step 1: Application Process for Enrollment
Both patient and physician will need to complete each section of the AAP application, sign and date, and return it by fax or mail for consideration to the Astellas Access ProgramSM. Please note, incomplete applications may result in a significant delay in the enrollment process.
In addition to completing the application, the patient will need to:
- Provide proof of gross annual household income, documented by a copy of the patient’s most recent tax return. If the household does not file taxes, Astellas Reimbursement ServicesSM may accept a copy of the patient's 1099 Social Security form, a copy of the most recent Social Security benefit letter, or a copy of recent paycheck stubs for a consecutive 30-day period.
Step 2: Enrollment Process
If accepted into the Astellas Access ProgramSM, both the patient and the provider will receive an acceptance letter confirming the start and end dates for the enrollment period. Upon approval, the patient will receive the first shipment of Amevive within 3 to 5 business days of his or her acceptance into the program. For subsequent shipments of Amevive, patients will be contacted by CVS Caremark to verify the correct shipping address. If the patient’s shipping address or prescription for Amevive changes at any time during the enrollment period, please call ARS at 1-800-477-6472.
Approximately 6 weeks prior to the end of the patient’s enrollment period, providers and patients will receive a reminder letter about reapplying to the Astellas Access ProgramSM. Please note, enrollment periods vary in length according to each specific patient situation; if enrolled in the Astellas Access ProgramSM the acceptance letter you receive will indicate the dates of your enrollment period.
