Prograf Value Card
The Prograf Value Card provides your patients with an instant rebate of up to $200 of their co-pay for up to 12 prescriptions at the pharmacy.*
Patients may receive the Value Card in one of two ways:
- Directly from their healthcare provider
- Online at www.transplantexperience.com, www.prograf.com, or www.astellastransplant.com
- Patients will be able to download and print a temporary card that can be activated immediately from the Web site
- Subsequently, a Value Card will be sent to the patient in the mail
Please note: The Prograf Value Card may not be used if the Prograf prescription is paid in part or full by any state or federally funded program including but not limited to Medicare or Medicaid, Medigap, VA, DOD or Tricare or where prohibited by law.
The enrollment process is simple. The patient calls the Prograf Value Card Support Line at 1-866-790-7659 and provides the following information:
- 9-digit number from their Prograf Value Card
- Name
- Last 4 digits of their phone number
- Zip code
- Date of birth
- Medicare/Medicaid status
Once the patient has provided this information, the Prograf Value Card will be activated. To receive the instant rebate, the patient simply needs to present the Prograf Value Card to the pharmacy.
Patients who receive their Prograf prescriptions by mail can also take advantage of the Prograf Value Card. Most mail order facilities accept the card. However, if the patient's mail order facility does not, he or she may utilize our mail-in option by filling out the Mail Order section of the Value Card brochure.
