Astellas Access ProgramSM Eligibility Requirements
In order for the healthcare provider to receive no-cost stock replacement of Astellas medications through the Astellas Access ProgramSM (AAP), the treated patient must:
- Have been administered an Astellas medication for an FDA-approved indication OR an authorized compendia listing; AND
- Have an income of less than or equal to 2.5X the Federal Poverty Level (FPL) see table below; AND
- Be uninsured
- Uninsured: a patient who has no health insurance; a patient who has health insurance that specifically excludes the Astellas medication; or a patient who has hit their annual benefit maximum. Patients awaiting determinations for public assistance (such as Medicaid) can be considered uninsured until such time as they are approved for public assistance.
For patients who meet the eligibility criteria listed above, the provider should complete the AAP request for assistance and submit it via Astellas eService.
2012 HHS Poverty Guidelines
The Astellas Access ProgramSM uses the Federal Poverty Level (FPL) as the basis for assessing financial eligibility for patient assistance. The FPL establishes an income threshold for poverty based on the number of people in a household and is updated every year by the Department of Health and Human Services. Patient income must be less than or equal to 2.5X the FPL to qualify for assistance for an Astellas product. The table below outlines the 2012 FPL for the continental United States, Alaska, and Hawaii, and the Astellas Access ProgramSM financial criteria (2.5X the FPL).
